Abstract

John Dong, the founder and CEO of Kaffee Kostuum developed the idea of Kaffee Kostuum in his own MBA capstone project five years ago. Shortly after graduation, he received seed money from business angels, as well as a favorable bank loan. His value proposition was clear from the beginning: “Be a provider of an unlimited variety of affordable suits, directly available from stock.” The idea sprang from his frustration with two less-than-ideal circumstances: He either had to wait four weeks to get a pricey tailor-made suit or purchase from among the limited selection of affordable suits in his local department store. To keep his company's prices down, Dong worked with a production unit in Vietnam.

Abstract

Global Connect, a major telecommunications service provider, partners with national cable providers to bundle media and telecom services offered through voice over Internet protocol (VoIP). Global Connect provides the VoIP physical infrastructure that enables cable providers to offer VoIP phone service to their end customers. VoIP cable services are growing at a faster rate than anticipated, leaving Global Connect incapable of meeting contractual agreements with the cable partners and preventing them from capturing substantial VoIP market opportunities. Students are asked to improve the configuration of work at this service organization by identifying the types of waste in the current process. Process improvements use lean tools and their impact is quantified using time and capacity analysis.

To view a service business as a process and to understand where to find the constraints regarding customer responsiveness (flow time) and sales (throughput). This requires a rather subtle capacity analysis.

Abstract

In this case, students assume the roles of FK Day and Dave Neiswander, leaders of the social enterprise World Bicycle Relief (WBR), which donates and sells bicycles in sub-Saharan Africa. As a social enterprise, WBR combines not-for-profit and for-profit activities. Starting as a traditional not-for-profit organization formed to donate bicycles after the Indian Ocean tsunami in 2004, WBR eventually added a for-profit arm to facilitate growth and reduce its dependence on donations and grants. As a result, by 2017 WBR had distributed around 400,000 bicycles, primarily to schoolgirls, entrepreneurs, and health workers. As the organization grows, its leaders are interested in optimizing operations and entering new countries in Africa. What is the optimal distribution of WBR's resources between its for-profit and not-for-profit operations? How should it define the objective of its operations: should WBR maximize its social impact or the total number of bicycles in the field? Which countries should it enter?

To answer those questions, students are required to analyze the social enterprise business model. This analysis starts at the strategic level and ties into the operational level. If desired, this analysis can be followed by an Excel optimization of WBR's operations. The case contains historical data on the organization and poses questions that can be analyzed from the perspectives of a number of academic fields. It can be used in various types of courses including strategy, not-for-profit organizations, operations, and finance. The instructor materials include a prepared Excel model that can be used to make the quantitative analysis accessible to students without quantitative backgrounds, videos from WBR, and a video that shows FK Day and Dave Neiswander answering questions in the inaugural use of the case at Kellogg.

Abstract

Purpose

Health care is an example of an organization where the needs of potential clients are much greater than the capabilities of the service delivery system. The implementation of any medical procedure, as well as the provision of any service, just like the manufacturing of any product, can be decomposed into a series of tasks. The purpose of this paper is to propose a model for measuring the effectiveness of quality assurance tasks in health-care delivery processes.

Design/methodology/approach

The authors analyze a system of factors that affect the implementation of tasks in a process. In their considerations, they have focused on four areas of science that describe conditions that are related to the implementation of tasks: Scheduling as a methodology for allocating resources to perform tasks; Capacity planning as a methodology for assigning values to given resources expressed by the number of tasks that can be executed with the resources; Queueing theory, used as a methodology for describing phenomena in which not all planned tasks are performed within the prescribed specification limits; and Quality management, as a methodology to ensure appropriate conditions for completing tasks (CCTs), where CCT is a representation of parameters of casual relationship between variables.

Findings

The authors show that the effectiveness of executing any scheduled tasks in the process is determined by the difference between the capacity of resources allocated (at a given time interval) and the number of tasks planned to be carried out at that time. The CCT conditions determine the level of capacity of the fixed amount of resources. It is shown that their deviation from the reference CCT specification may cause the nominally correct amount of resources be either too small (causing queue formation and longer wait time in hospitals) or too large to contribute to the waste in the system by creating idle capacity.

Practical implications

The scope of application of the model is wide. It covers tasks performed with different degrees of uncertainties regarding the capacity of resources. It applies in all areas of health care where unlike manufacturing, the services delivered and the tasks performed in the health-care delivery system are seldom identical. Every patient is treated differently than the one waiting next in line. The workloads are pre-arranged in the order they are needed and completed in accordance with the FI-FO (first in-first out) principle. The model presented in this paper makes it possible to better understand the mechanism of effectiveness and efficiency improvement and the role of humans as a specific carrier of capacity.

Originality/value

As most of the health-care organizations are still stuck in the soft side of quality assurance, there has been little research conducted to test the applicability of well-known productions/operation management methodologies and theories benefitting health-care systems. The formulation of a reference point of CCT in this study is to serve as a stabilizing control point with the same connotation as that of a central reference line in the statistical process control chart. The correct capacity planning is needed to determine with a high degree of probability of success in implementation of all tasks to assure quality all the time.

Abstract

Purpose

The purpose of this paper is to understand the implications of outsourcing at the operational planning level, i.e. how the operational planning function is complicated owing to the strategic outsourcing decisions that have been made in the past.

Design/methodology/approach

First, a literature review on outsourcing is conducted. Second, two case studies at three pharmaceutical companies are conducted to gather insights into the planning of outsourced operations.

Findings

The paper finds that nothing has been documented in the literature on outsourcing at the operational planning level. Moreover, a number of implications of outsourcing at the operational planning level are discussed. One of the main insights is that in an outsourcing relationship, the order process consists of different, hierarchically connected, decisions in time, hence requiring a richer and more developed communication and ordering pattern than is commonly assumed.

Research limitations/implications

The results seem to be generalizable to the pharmaceutical industry. However, future research should determine whether these results replicate in other industries.

Originality/value

The literature on outsourcing has mainly focused on the strategic outsourcing decision. The paper contributes to a better understanding of the implications of outsourcing at the operational planning function, which has not been studied before.

Abstract

Purpose

The purpose of this study had two aims: (1) to extend insight regarding the challenges of implementing standardised work, via care pathways, in a healthcare setting by considering interactions with other operational (i.e. resource sharing, portfolio alignment) and professional (i.e. autonomous expertise) dependencies and (2) to develop novel insights regarding a specific flow mechanism, the stroke nurse practitioner, a form of flow “pilo” or guide.

Design/methodology/approach

This was a longitudinal case study of implementing the acute stroke care pathway in a National Health Service hospital in England based on 185 hours of non-participant observations and 68 semi-structured interviews. Archival documents were also analysed.

Findings

The combined flow, operational and professional dependency lens extends operations management understanding of the challenge of implementing standardised work in healthcare. One observed practice, the process pilot role, may be particularly valuable in dealing with these dependencies but it requires specific design and continuous support, for which the authors provide some initial guidance.

Research limitations/implications

The research was a single case study and was focussed on a single care pathway. The findings require replication and extension but offer a novel set of insights into the implications of standardised work in healthcare.

Originality/value

In addition to confirming that a multidependency lens adds conceptual and practical insight to the challenges of implementing standardised work in a healthcare setting, the findings and recommendations regarding flow “pilots” are novel. The authors' analysis of this role reveals new insights regarding the need for continued improvisation in standardised work.
